Changes to registration during Covid-19 Pandemic

Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online registration form


New Patient Registration

If you wish to register with Lakeside, please collect a Registration Pack from Reception for each member of the household. You will need to complete the Registration Forms and return them to us together with the documents shown on the form.

Forms must be brought to the Health Centre IN PERSON – we cannot register any adult patient without them attending the practice themselves at the time of registration.

Please note that you do NOT need any identification documents (e.g. passport or visa) to register with the Practice – we only need to see recent proof of address to show that you live within our area.

When you have supplied your details you will be asked to make an appointment with one of the nursing team to have a New Patient Check. 

You are not fully registered until you have attended your new patient appointment as it helps us to assess your ongoing needs. 


Medical Card

If you are new to the NHS, your letter of confirmation will take between 2 and 3 weeks to arrive from the Health Authority.


Named GP

We are now required under the General Medical Services  contract to inform all of our patients who their named, accountable GP is. This is usually your registered GP, although you can request to see any doctor in the surgery.
